5"d, white, blank up kit, screw holes 2-3/4" apart, decorative piece used to cover the outlet box after removal of an existing ceiling fixture, uses a crossbar lockup, securing the blank up to the crossbar with screws & cap nuts, includes 1 each of the following: 5" blank up canopy & offset crossbar & 2 each of the following: 8-32 x 1-1/2" mounting screws & brass cap nuts.
